Use three numbers, not one
A provider record may show a billed charge, amounts paid or adjusted, and a stated balance. Those figures can differ. Enter them in separate columns and attach a source date. The ledger’s arithmetic only helps spot mismatches; it does not decide the legally recoverable amount or the amount a provider, plan, or program may claim.
- Billed: what the statement lists as charges
- Paid: documented payments from identified sources
- Balance: the amount currently stated as remaining
